Student Health

The Student Health Office was developed to:
  • Keep all students’ health information centralized in one location.
  • Assist in communication between students, parents, administrators, teachers, counselors and coaches in matters pertaining to a student’s physical health.
  • Notify administrators, teachers, counselors and coaches as needed when a student needs academic adjustments due to injury or illness.
Student Health information is currently being updated for the 2024-25 school year. Please check back after May 1, 2024 for all the information and forms for the next school year.

The health and wellness of our students and families is a top priority, and Loyola High School is committed to promoting the physical and mental well-being of our students.

There are four mandatory health requirements for ALL students that must be completed at the beginning of every school year: 1) Annual Physical Exam; 2) Annual Concussion Baseline; 3) Current Immunization Forms; and 4) Online/E-Signature Forms. Instructions and descriptions of each category can be found under each named tab on this Student Health page.

ALL PHYSICALS MUST BE PERFORMED AND DATED AFTER MAY 1, 2024 TO BE VALID FOR THE 2024-2025 SCHOOL YEAR.

ALL students at Loyola High School are required to complete an annual physical exam.

Please read all instructions
There are several ways that you can complete the annual physical requirement:
1. Download the physical form and take it with you to your doctor appointment.
a. Schedule an appointment with your primary doctor.
OR
b. There are numerous Urgent Care Clinics and CVS Minute clinics located throughout Los Angeles that will also perform physicals.
OR
c. Loyola has partnered with Exer Urgent Care to have physicals performed at no cost. You MUST read the information on this link in order to take advantage of the service.
d. Loyola High School recommends that you ask your medical provider about having an EKG performed during a physical exam. This is NOT a requirement.
2. Once you complete the physical, PLEASE make sure the Medical Provider’s Office Stamp is on the form. The form will be returned unless the Medical Providers stamp is on form.
3. Please make a copy of the completed form for your own records. DO NOT LEAVE FORM WITH DOCTOR’S OFFICE FOR THEM TO MAIL.
4. Below are the various ways to turn in completed form:

– Submit form through our student health website, www.swol123.net. Under the “FORMS” tab you can select “ADD” and upload the form. Instructions on how to log in to www.swol123.net are found here.

– Drop Off at Loyola’s main office-located in Loyola Hall or my office located in Leavey Gym.  DO NOT GIVE IT TO A COACH OR TEACHER.

Once Loyola receives the completed physical form (PPE) you will receive a confirmation email. If you do not receive a confirmation email within 24 hours, please contact the student health office at studenthealthadmin@loyolahs.edu.

IMPORTANT DUE DATES
Incoming freshmen and/or new students:
– If your son is a new student at Loyola and planning on participating in the summer weight training program or a Loyola Athletic team,  a completed physical form is required before the first class meeting or practice, June 17, 2024.
Returning Students:
– If your son is planning on playing a Fall Sport – Football, Water Polo, Cross Country – physicals are due by July 29, 2024.
– All other students – physicals are due the first day of school August 12, 2024.

California State Law mandates that all students attending school MUST show proof of immunization. Your previous school should have a copy of your completed immunization. Loyola High School has already asked for these documents to be sent with your final transcripts. However, it is still your responsibility to make sure we receive your up to date immunization form. You will not be allowed to attend school until we receive your immunization form.

Forms can be submitted the following ways.

Once Loyola receives your immunization form, you will receive a confirmation email.

Please follow ALL instructions to schedule an appointment for a Concussion Baseline.

IMPORTANT INFORMATION
ALL students are required to complete a concussion baseline, whether or not they participate in athletics.
DO NOT pick more than one time slot per student, we have several people performing baselines and can accommodate several students at a time.
– You cannot schedule a baseline during your summer school class.
– Baselines take approximately 10 minutes to complete.
– Baselines are performed at the Hannon Learning Center/Library.

Register
– Enter your E-mail address and create a password. 
– Then click on the “Login/Create Account” button.
– Fill in the required information to register. Please use STUDENT’S NAME when scheduling an appointment.  
– Click the “OK” button to register
– Once you have registered and clicked the appropriate window, you will see all available appointment times. Only open appointment times will show. If no times are showing, that means all times have already been booked for that particular slot.
– If a time slot is available, the time will be showing. To select your time, simply click on the time that you want.
– Please only pick one time slot for a student.
– You can choose to have a reminder email sent to you prior to your appointment.
– Click on the “Create Appointment” button. This will confirm your appointment for the date and time you selected.
– To end your session, select the “Logout” button.
– You will receive an email confirming your appointment

If you are a incoming Freshman or new student please do NOT attempt to login to Sportsware until after June 1, 2024

Online Instructions for Sportsware
  • All Loyola Parents and students are required to complete
  • 1. Authorization forms
    2. Update demographic information
    3. Complete health history
    ALL of the above forms/documents are completed online in the Sportsware Program.

  • Loyola High School uses an online data entry system called SportsWare. This system is secure and HIPAA compliant. Please refer to the instructions below to complete the online process.
  • Some E-forms require an E signature are clearly identified in parenthesis as either “ALL STUDENTS” or “ATHLETICS” ALL students, while others are for students planning to participate in athletics.

    HOW TO LOGIN TO SPORTSWARE

    1. Go to www.swol123.net
    2. In the upper right corner you will see a box labeled “Sign In”
    3. In the box labeled “email” **
    a. If you are an incoming freshmen/new student enter the student’s mother’s email that was used during the enrollment process.
    b. If you are a returning student, enter the email that was used last year. **PLEASE NOTE THERE ARE TWO PORTALS TO THE SITE ONE PORTAL IS FOR PARENTS AND THE OTHER IS FOR STUDENTS/ATHLETES. TO COMPLETE THE ONLINE FORMS YOU NEED TO BE IN THE PARENT PORTAL SECTION.
    4. In the box labeled “Password”
    a. If you are an incoming/new student you need to click on the “reset password/forgot password” under the “LOGIN” box. A password will be sent to the student’s mother’s email that was used during the enrollment process.
    b. If you are a returning student you can enter the password you used from the previous year, or if you forgot the password, click on the “reset password/forgot password” under the “LOGIN” box. A password will be sent to the email you used last year to login.

    Once you login to the program you can change the default email and password. You cannot use a student’s email in the “Parent Portal” section.

    Once you are logged into Sportsware follow instructions below:

    Athlete Portal – Parent

    Once you login you will be directed to your dashboard page.
    – The Page should have “ATHLETE’S PORTAL – PARENT” across the top.
    – Parent’s YOU need to fill out the necessary requirements here.
    a. Your son will need to e sign three forms under the “Forms” tab.

    There are THREE sections that are completed by parents.
    1. Click the “My Info” tab and verify or enter all the information that has a “RED” asterisk next to it. Located in left column on page. Make sure to open each section and complete information required.
    2. Click the “Med History” tab and answer all questions about your son’s health history. Located in left column on page
    3. Click the “FORMS” tab. Located in the left column on the page. Please select each form that says Parent’s Signature Required. These forms are signed electronically.
    a. This is also where there are three forms that your son needs to sign.
    PLEASE DO NOT ATTEMPT TO DOWNLOAD AND MAIL THE FORMS.
    Below is a brief description of each form.

    PARENT FORMS

    Consent to Treat (ALL STUDENTS)– Electronically signed by parent.
    Concussion Information Parent(ATHLETICS)- Electronically signed by parent.
    Catastrophic Information for Parents(ATHLETICS)– Electronically signed by parent.
    Parent Opioid Information(Athletics)- Electronically signed by parent.
    Over the Counter Medicine Consent(ATHLETICS)- Electronically filled out and signed by parent.
    Sudden Cardiac Arrest Information Parent (ATHLETICS)- Electronically signed by parent.

    STUDENT FORMS

    Athlete Portal – Minor Athlete

    Students once you login to your portal you will be directed to your dashboard page. The Page should have “ATHLETE’S PORTAL – Minor Athlete” across the top. Student’s YOU need to fill out the necessary requirements here.
    There are Two Sections completed by Students:
    1. Click the “My Info” tab. Located in the left column on the page. Complete the required fields.
    2. Click the “PHQ-9” tab, located in the left column on the page. Hit the “+ADD” button on the right side of the page. Answer ALL questions. Answers are confidential.

    Once you complete all the necessary forms and updates on the dashboard page you should see the phrase “Your Athlete Information is Complete.

    If you see the phrase, “Your athlete information is INCOMPLETE”. Please click the link on the page and complete the required fields.

    PARENTS
    You can view your son’s health requirement progress by clicking on the “PAPERWORK”. Once a requirement is completed you will see a box with the date of completion of that step. Please disregard the Game Status and Practice Status comments.

    PLEASE NOTE: You will see 2 boxes labeled in this section, “IMPACT BASELINE 2022” and “IMPACT BASELINE 2023”.

  • If you see a date in the 2022 box you do not need to complete this baseline this year.
  • If you do NOT see a date in the 2023 box you must complete this baseline and then you will see a date in the 2023 box.
